Disability Transition Support Officer - Box Hill Institute
Employment information: Full-time, fixed-term opportunity until February 2028;,$87,524 plus 12% superannuation

As a Disability Transition Support Officer, you'll provide personalised guidance, advocacy and intensive transition support to students, families and schools, helping students build confidence, access appropriate support and successfully commence their studies at Box Hill Institute.
Duties
- Provide individualised transition support to students with disability
- Develop transition plans and coordinate tailored support strategies
- Assist students and families to navigate enrolment and student support processes
- Facilitate meetings with schools, families, allied health professionals and Institute staff
- Collaborate with internal support services to improve student outcomes
- Build relationships with secondary schools and external disability support networks
- Maintain accurate records, reporting and program data
- Contribute to best-practice disability transition initiatives across the Victorian TAFE Network
Skills and Experience
- Experience supporting people with disability, mental health conditions and/or medical conditions
- Understanding of disability-related legislation, reasonable adjustments and inclusive practice
- Excellent communication and stakeholder engagement skills
- Strong case management, planning and organisation abilities
- Ability to build effective relationships with students, families, schools and internal stakeholders
- A commitment to improving educational outcomes for people with disability
- Experience within education, disability, youth, community services or a related sector will be highly regarded
Qualifications/Requirements
- Relevant tertiary qualifications and/or significant experience in disability, education, community services or a related field
- Demonstrated understanding of disability support frameworks and relevant legislation
- Current Employee Working With Children Check
- Current National Police Check
