To assist students with disabilities identify the services and study supports they may need and to negotiate
reasonable adjustments
with academic staff, disability staff compile a document called an
Access Plan. This is required for all students that choose to access disability
related supports while studying. The plan is compiled in conjunction
with the student and is tailored to meet their study needs. Each student
is provided with a hard copy of their Access Plan and a copy is
generally sent to the Program Director for information. Examples
of the information contained in an access plan include:
- the impact of a student’s disability and/or medical condition on their
course of studies
- services provided to the student by the tertiary
institution
- any agreed variation to exam arrangements
- information
provided by the student to the Disability Service
- information provided by
any relevant service providers
- documentation provided by the student from
an appropriately qualified practitioner regarding their disability and/or
medical condition
- information provided by relevant tertiary staff
Listed below are three examples of an access plan used
